The commission shall create a permit which shall identify the person or entity involved in the filming or televising including the address and telephone number and shall identify the locations of the filming and the dates and approximate duration of the filming and the nature of the filming and the existence of any special effects or use of animals or firearms and the existence of liability insurance and such other information as the commission feels is necessary for the protection of the public and the public welfare.
